Certain services offered in connection with the Slim + Contour brand are provided by a physician-owned professional corporation in accordance with California law. The Medical Practice is a HIPAA-covered entity and is responsible for maintaining the privacy and security of your protected health information.

Protected health information may be used or disclosed for treatment, payment, and healthcare operations, or as otherwise permitted or required by law.

Contour Science LLC provides non-clinical management, marketing, and administrative services and does not control medical decision-making.

You have the right to:

• Access and obtain a copy of your medical records

• Request corrections to your medical information

• Request restrictions or confidential communications

• Receive an accounting of certain disclosures
